01 The challenge

When a patient was transferred between hospitals, the receiving team often had no access to prior records. Tests were repeated, decisions delayed and safety risks logged. Each EHR vendor had its own data model and interface, and any solution had to meet NHS information-governance, security and audit requirements from day one.

02 What we did

  • FHIR-compliant integration layer normalising patient, encounter, medication, allergy and results data from every source system
  • Master patient index matching identities across sites safely
  • Clinician-facing record viewer embedded in existing workflows, returning a full history in seconds
  • Role-based access, consent handling and full audit trail aligned with NHS governance
  • Hospital-by-hospital onboarding with clinical safety sign-off at each stage

  • Every hospital in the trust connected within the programme timeline
  • Duplicate investigations reduced substantially
